- Registration window: Opens May 11 and closes May 22. One submission per athlete.
- This is a request only — submitting the form does not guarantee an appointment. Space is very limited.
- Confirmation required: Your spot is only secured after you receive a confirmation text from our team.
- No walk-ins: We are unable to accept appointments without prior scheduling.
- Submitting your forms: It is your responsibility to submit any school-required forms (standard Florida DH 3040 / EL2) to your school after the event.
Florida School Health Physical Disclaimer: By registering for this event, I understand that the physical examination provided is a general school-required health screening and does not replace regular care by a primary care physician. I confirm that I am the parent or legal guardian of the child(ren) being registered and that I give consent for my child(ren) to receive a school-entry physical exam from a licensed medical professional. I understand that all information collected during this event will remain confidential and used solely for the purpose of school health requirements.
I acknowledge that it is my responsibility to bring any necessary forms required by my child's school, and that Redemption Orthodontics and its partners are not responsible for verifying school-specific requirements beyond the standard Florida school entry physical form (DH 3040, EL2).

Who Can Register
This event is designed for middle school student athletes preparing for the 2026/2027 school year.
Rising 6th Graders
Students entering 6th grade in the 26/27 school year are eligible to register.
Rising 7th Graders
Students entering 7th grade in the 26/27 school year are eligible to register.
Rising 8th Graders
Students entering 8th grade in the 26/27 school year are eligible to register.
Rising 9th graders and other high school grades require an EKG for the 26/27 school year and are not part of this event. A waiting list option is available on the form.
